Alzheimer's and Individuals with Special Needs Assistance Initiative

As part of our mission to protect life and to assist the public-at-large in a manner consistent with the rights and dignity of all persons, the Grantham Police Department offers a program for individuals with Alzheimer’s and Special Needs to rapidly respond in the event that an individual wanders off, becomes lost or has a medical emergency.

If you are the caregiver for someone living in Grantham who has been diagnosed with Alzheimer, or an elderly person who tends to wander or get lost, or an individual with special needs, please fill out the attached information sheet and contact the Grantham Police Department at (603) 863-6844.  This simple, one page form includes information that will help us identify the person, such as height, weight, hair and eye color, as well as provide names of emergency contact people and medical information such as preferred hospital, primary care physician, allergies and medications.  It is helpful to have a photograph of the person. 

 All of the information you provide will be entered into our computer system along with the photograph. 

 If the individual becomes lost, call the Grantham Police Department immediately (863-6844 or dispatch at 863-3232) and advise that the individual is a participant in the Program.
